Use this service to cancel an appointment. Let us know as soon as possible if you cannot make the appointment.
If you are cancelling an appointment which is within the next 24 hours, or if you are completing this form over the weekend for an appointment on a Monday, you must phone the surgery on Cuffley Village Surgery 01707 875201 or Valley View Health Centre 01707 875223 and press option 3 to leave a recorded message to cancel your appointment. You can only press option 3 when the surgery is open. If you are phoning after 8:30pm or at the weekend, please phone 07470 773 298.
